Clinic

• Medication at school:  Conroe ISD policy allows the administration of medication to students during school hours should it be necessary for the student’s optimum health.  The school clinic maintains information on file for all students who receive their medication.  The registered nurse, clinic assistant, administers the medication.
• Non – prescription drugs and prescription drugs:  The school must receive a written, dated request from the parent or legal guardian to administer any medication. This permission must include the name of the medication, exact dosage the student is to receive and the reason for the medication to be given at school.  Medical Permission Sheets are available in the clinic for parents to fill out.
• Prescription and non-prescription drugs must be in the original container, properly labeled, approved by the Federal Drug Administration, non- expired, and age appropriate.  The Surgeon General, the Food and Drug Administration and the Center for Disease Control have recommended that due to the increased risk of developing “Reye’s Syndrome”, aspirin or products containing aspirin not be given to children under 18 years.  If your physician orders aspirin for your child, please send a copy of the physician order.
• No products containing aspirin will be administered at school without a written order from your physician.
• Additional guidelines for medication at school:  All medication must be kept with the school nurse in the clinic during the school day.
• Over – the- counter medication:  dosage must not exceed the recommendation listed on the bottle.  Exception:  If your physician, dentist or orthodontist has directed a certain dosage to be given that is greater than that recommended on the bottle, a written order would need to be included with the written permission from the parent.  The use of “sample” medication from the physician, dentist, or orthodontist must have signed written instructions from that doctor accompanied by the parent’s written permission.
• In the interest of safety for all students, medications cannot be transported to or from school, please bring medication to the clinic.  For your convenience, many of the local pharmacies will provide a second labeled container for medications needed at school.
• Inhalers:  Students that have asthma may experience times when symptoms worsen and the physician request the student to carry an inhaler to be used when needed.  Please provide a letter from the physician permitting the student to carry the inhaler.  It is strongly recommended that a spare inhaler be kept in the clinic in case the student cannot locate the in haler.

House Bill 1688 entitles students with Asthma to possess and self administer their prescription asthma medication while eon school property.

• Exercise – induced Asthma:  Students in physical education classes may keep an inhaler in the clinic to use before PE, or they may come to the clinic with an escort during PE.  The clinic can also send an inhaler to athletics so the student will have access to their inhalers during practices, home games and away games.  A second inhaler in the clinic is recommended for before and after practices.  A CISD School Asthma Actin Plan must be on file with the school nurse in the clinic.  This form must be completed at the time of enrollment in school or at the beginning of each school year.
• Anaphylaxis medication:  House Bill 1 (passed by 2006 Texas Legislature and effective beginning 2006-2007 school year) entitles students to possess and self-administer anaphylaxis medication while at school.  Article 10 states that this procedure must be approved by a physician or licensed health care provider.  The student must demonstrate to the physician or licensed health care provider and the school nurse the skill level necessary to self – administer the medication.  It is recommended that an extra dose of medication be kept in the clinic.  A CISD Authorization for use of Anaphylaxis Medicine must be completed and be on file with the school nurse in the clinic.
• Diabetes Medical Management Plan:  House Bill 984 entitles students with Diabetes to develop a management and treatment plan to be followed while the student is at school.  A CISD Diabetes Medical Management Plan and Individualized Diabetes Health Plan must be completed and signed by student’s parent or guardian and physician responsible for the student’s diabetes treatment.  The plan must identify the health care services the student may receive at school, and evaluate the student’s ability to manage and understand their diabetes.  The plan must be submitted to and reviewed by the school.
• All medication is to be picked up in the school clinic by a parent or guardian at the end of the school year.  No medicine will be kept over the summer months to be used the next school year.

• If a student requires medication from any ailment or injury for longer than two school weeks, a prescription or doctor’s order is needed for both prescription and over-the-counter medication.
• If a student requires crutches, a doctor’s order is required.  Students must present doctor’s order to the clinic upon arrival to school.

All injured students that require a buddy to escort them from class can be dropped off and picked up at the front of the building.

• Buddy passes are given through the clinic.  With a buddy pass, the injured student will have an escort to and from classes while the hallway is not occupied.  The buddy will help with carrying of all school materials.
• Early Release Only passes are given through the clinic.  The pass is given to those students who are in need of a few extra minutes in the empty hallway to get to class.  A signed note from parents/doctor is required.
• Passes are given expiration dates.  To renew a pass, proof of doctor visit is required

If going out of town we need the following information:

• Who is in charge of your child
• Duration in charge of your child
• Telephone numbers home, work, cell
• Your signature

Conroe ISD follows the Texas Dept. of State Health Services recommendations for exclusion from school and readmission.

Additionally, students who present with the following issues will be sent home from school:

• Temperature of 100 degrees or higher
• Diarrhea
• Vomiting
• Suspected contagious illnesses
• A child who is too ill to function in the classroom

A child with a fever may return when their temperature has been BELOW 100 degrees for at least 24 hours without the use of fever reducing medication (Tylenol, Motrin, Ibuprofen, etc)
