Be sure to notify us of telephone number changes - home, work or cell phone. We like to get in touch with you in a timely manner, and the students can't always remember new number
If going out of town we need the following information:
1. Who is in charge of your child
2. Duration in charge of your child
3. Telephone numbers home, work, cell
4. Your signature
If a tetnus booster (10 year booster) is past due, your child will not receive his/her Fall schedule at the school they are attending the next year: Knox or McCullough
Please take documentation of shots received to the clinic in the school your child will be attending
